Fall Prevention
Falling is very common amongst older people and is responsible for considerable disability and distress. Whilst interventions such as exercise aimed at improving balance and increasing muscle strength can reduce falling amongst older people this evidence has not successfully been rolled out into useful treatments taken up by older people. No one has yet demonstrated fall prevention programmes can reduce the numbers of fractures experienced by older people.
The Prevention of Falls Network Europe led by Chris Todd co-ordinates research across Europe into the prevention of falling amongst older people. This work co-ordinates trials of fall prevention, identifies best practice treatment regimes, investigates assessment of balance and gait, and focuses on psychological consequences of falls, and psychological processes which motivates older people to take up fall prevention activities. In particular Chris’ group have been investigating how to make fall prevention, attractive to older people by investigating their beliefs and attitudes towards falls and exercise. They recently devised a new measure of fear of falling to be used across Europe. In addition they are investigating the effect of social deprivation and ethnicity on the use of services aimed at reducing falls.
Palliative Care
Many people with cancer and other life threatening illnesses would prefer to die at home, but many who prefer to remain at home fail to do so. A major area of interest is how to support people during their last illness so that they can have maximum quality of life and remain with their family and friends. Chris’ group have been investigating access to palliative care services and the way that community based palliative care services, nursing services and social care, can help people to remain at home when they are seriously ill. An important issue is the way that services are organised and the training and skill mix of professional providing those services.
However patient and family factors are not clearly understood and they have been looking at patient and carer variables involved in continued patient support. Some of the most needy patients fail to access services which could help them and their families during their final illness. The group are investigating ways of improving access to services and how novel interventions can help. These include access to complementary therapies and they are investigating what patients expect and their experiences of such services.
